Identified
The source of the problem has been identified and a fix has been rolled out. Once applied a backfill of all affected responses will be run.

Investigating
We are investigating an issue where some results and responses are not fully processing. All response data is being captured and stored correctly; however, it is not currently flowing through our aggregation and reporting systems.
